I will give a brief overview of this topic.<\/p>\n\n\n\n<p>1.\u00a0\u00a0 <strong>What is it?<\/strong><\/p>\n\n\n\n<p>It is a disruption of the joint capsule of the major shoulder joint (the glenohumeral joint).<\/p>\n\n\n\n<p>2.\u00a0 <strong>What causes it?<\/strong><\/p>\n\n\n\n<p>It is caused by a sudden jolt to the shoulder joint, usually in a rugby tackle or heavy fall. The socket of the shoulder joint (glenoid) is quite shallow, so the shoulder is not as stable as other joints like the hip.<\/p>\n\n\n\n<p>3. <strong>Symptoms <\/strong><\/p>\n\n\n\n<p>\u00a0 What you notice<\/p>\n\n\n\n<ul>\n<li>\u00a0Pain &#8211; because of tearing of the joint capsule, there is severe pain when it first occurs. Recurrent dislocations are usually less dramatic.<\/li>\n<li>Stiffness &#8211;\u00a0 when the humerus (upper arm bone) is out of the socket, movement is very restricted.<\/li>\n<\/ul>\n\n\n\n<p>4.\u00a0\u00a0 <strong>Signs <\/strong><\/p>\n\n\n\n<p>What the doctor finds<\/p>\n\n\n\n<p>Deformity: there is usually a loss of the usual prominence of the humerus (upper arm bone). In very muscular people, the deformity may be hard to see.<\/p>\n\n\n\n\n\n<p>\u00a0 Tenderness: over the torn tissue, or other tissue that is under extreme tension.<\/p>\n\n\n\n<p>\u00a0 Stiffness: shoulder movement is very restricted.<\/p>\n\n\n\n<p>Occasionally there may be numbness of an area of skin over the shoulder, plus weakness of shoulder movement. This suggests that the axillary nerve around the shoulder has been stretched during the dislocation.<\/p>\n\n\n\n<p>5. <strong>Investigations\/tests<\/strong><\/p>\n\n\n\n<p>X-rays can show the dislocation, plus any associated bony injury, e.g. a fracture of the rim of the socket (glenoid rim fracture or Bankart lesion). If surgery is being considered, an MRI scan may be ordered.<\/p>\n\n\n\n<p>6.\u00a0 <strong>Treatment<\/strong><\/p>\n\n\n\n<p>a. Reduction of the dislocation. This should be done as soon as possible, by the best qualified person available. First time dislocations usually need reduction at an A&amp; E clinic or hospital but repeat dislocations can often be put back on the sideline.<\/p>\n\n\n\n\n\n<p>b.\u00a0 A sling for four weeks provides support and helps ease the pain.<\/p>\n\n\n\n<p>c.\u00a0 Pain relief with paracetamol or Voltaren or similar medications.<\/p>\n\n\n\n<p>d.\u00a0 After four weeks a series of muscle exercises from an experienced physiotherapist can help restore normal strength and mobility.<\/p>\n\n\n\n<p>e.\u00a0 Surgery may be required to repair torn tissue. The chance of redislocation is higher in young\u00a0athletes playing contact sports like rugby. The decision to have surgery needs to be carefully considered between the patient and their surgeon and depends on many factors. Any person having surgery needs to commit to a rehabilitation programme lasting several months.<\/p>\n\n\n\n\n\n\n\n\n\n<p>7<strong>. Recovery time<\/strong><\/p>\n\n\n\n<p>Average recovery time is 2-3 months without surgery. If surgery is required, depending on the complexity of the procedure, recovery time is between 4 and 12 months.<\/p>\n\n\n\n<p>8.Recovery sequence<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 1<\/span><\/p>\n\n\n\n<p>Reduction of dislocation.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 2<\/span><\/p>\n\n\n\n<p>Sling and painkillers.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 3<\/span><\/p>\n\n\n\n<p>After four weeks start gentle passive exercises.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 4<\/span><\/p>\n\n\n\n<p>As range of movement increases, move onto strengthening and stability exercises using theraband and weights with pulleys.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 5<\/span><\/p>\n\n\n\n<p>Restart gentle contact training when cleared by your physiotherapist or doctor.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Step 6<\/span><\/p>\n\n\n\n<p>Continue a maintenance exercise programme to keep your rotator cuff muscles.<\/p>\n\n\n\n<p>We have noticed that in Nigeria most patients do present late with recurrent shoulder dislocation having had more than 2 episodes. For this group of patients, surgical stabilization is usually the preferred modality of treatment.
